Graduating Senior Refund

By submitting this form and accepting a refund you understand that the recipient forfeits eligibility to compete or participate in any future event(s) produced by In10sity Dance.

All checks will be made out to the dancer’s studio.

Instructions:

ONE dancer per form.

Submit a separate form for each dancer.

Please note before you submit this form:  The “senior” age division will be expanded to ages 15-20 in the 2021 season to accommodate those dancers affected by the shortened 2020 season.

This form will remain available throughout the 2021 season for those seniors who are, as of now, still undecided about their 2021 plans.
